REFLECTOR: Unexpected door opening

On the surface modifying the door latch to improve security seems to be a good idea.  Rescue in an emergency situation has always lead me to a have a bit of concern.  Assume a pilot has an incident and is incapacitated, the incident has left the AC inverted and the pilot side door is blocked.  The factory system leaves no method, short of a fire ax, to open the copilot side door from ground rescue.  On mu AC, I have followed the factory system, except that I added the external release to the copilot side (and I hope it will never be needed.)

How many open door incidents have occured using the factory system?  Is a better warning system for an improperly secured door needed, such as limit switches on all latching pins?
